scripts/db/create-app-account.sh creates or updates the account named by
DB_USERNAME with DB_PASSWORD and grants it the rights the server needs on
the project schema. With --rotate-root it also aligns root on
DB_ROOT_PASSWORD, which an already initialised MariaDB volume ignores on
container recreation. Values travel through stdin and MYSQL_PWD only.

- Backend DATABASE_URL now uses DB_USERNAME/DB_PASSWORD instead of root,
  in the dev and prod compose files and in the env templates.
- MariaDB healthchecks no longer pass the root password as an argument
  (healthcheck.sh in dev, unauthenticated mysqladmin ping elsewhere).
- The rust-test service no longer points TEST_DATABASE_URL at the dev
  database: the ignored tests delete data and need a disposable one.
